Skilled Nursing Care

Our emphasis on excellent clinical care allows us to have an array of professionals to care for your loved one. Each floor is staffed on the day shift with a dedicated Nursing Supervisor, a Charge Nurse who could be either a Licensed Practical Nurse (LPN) or a Registered Nurse (RN), a Certified Medication Technician (CMT) and Certified Nursing Assistants (CNAs). Thanks to their skill and dedication, we are equipped to care for residents with wounds, tracheostomy tubes, g-tubes, IVs, catheters, nephrostomy tubes, and more.

As your loved one’s needs for greater care increases, we are able to adapt and provide the highest level of clinical care. With constant attention on our residents, our Mission Partners work together to provide the best level of care. All our rooms are capable of the highest level of skilled nursing care, so your loved one can remain in the same room as they move to greater dependency.

Mother of Good Counsel Home enjoys a generous nursing staff structure that significantly exceeds national staffing benchmarks for skilled nursing. Higher staffing levels are an intentional decision by the Home, since we believe that resources should be prioritized toward the care of our residents. This supportive staffing structure creates an environment where our Mission Partners want to work, resulting in strong Mission Partner commitment to our Home and staff longevity. Consistent staffing then drives optimal care for our residents, where Mission Partners and residents can form lasting and trusting relationships.

In each room and restroom, our emergency call response system allows for residents to receive help promptly. Our Mission Partners are equipped with phones that allow them to talk to residents even when in another area of the nursing floor. This allows for triage, prioritization, communication, and the safest care possible.

Our team comes together on a regular basis with residents and/or their family to discuss all aspects of their life here at the Home. These discussions help us adapt and evolve our care plans for each individual to ensure our care meets their needs and helps them to flourish. We want each person to feel heard, seen, known, loved, and cared for. You will see the following people at our care plan meetings: clinical nursing supervisor, social services, food service manager, activities supervisor, therapy representative, among others.

As one ages, the ability to perform daily tasks begins to take more and more energy and strength. We are grateful to be able to provide daily assistance for our residents while promoting their current level of independence and strength. With the strong foundation of our dedicated CNAs, our residents are cared for in everyday tasks and immediate needs. The team truly works together so that each resident can receive the attention and love they need in order to flourish.

Rehabilitative Services

EmpowerMe Wellness is an excellent clinical partner, providing Physical Therapy, Occupational Therapy, Speech Therapy, with an overall wellness approach for our whole resident family.

When residents no longer qualify for professional therapy services covered by insurance, they may access the Home’s restorative services, which involves a dedicated Mission Partner who helps residents maintain a regimen of exercise and physical movement with the goal of maintaining strength and current level of activity.

Additional Clinical Services

The Home is supported by several geriatric-specialized physicians who manage the medical needs of our residents. With a committed onsite presence several times a month, our physicians lead the medical care of our multi-disciplinary team of caregivers.

The Home has a dedicated pharmacy relationship, in which medications are delivered to the Home several times a day, including overnight deliveries. There is also a pharmacy-managed onsite emergency supply of commonly-needed medications for new orders that require more immediate administration.

The Home works with three established hospice companies that reflect our values and offer expert end-of-life comfort and care. With consistent hospice partners, we maintain good working relationships between our team and theirs, thus serving our residents with coordinated and seamless care. Hospice also provides specialized equipment and supplies covered by Medicare.

“Food is medicine,” especially in the later years of life. Our registered dietitian adds an important component of ensuring we have well-rounded plans of care for the flourishing of our residents.

A licensed podiatrist visits the Home on a regular basis for all of the foot-care needs of our residents, especially safe toenail maintenance.

Experts in ear care and cleaning, our audiology services can help our residents hear better! They are able to help with maintenance of hearing aids, but they do not have onsite equipment for hearing evaluation needed for prescribing new hearing aids.

Professional optometry services visit the Home regularly in order to fix and adjust eye glasses for perfect fit, perform eye and vision evaluations, and order eye glasses for our residents as needed.

A professional dentistry service performs onsite cleanings, evaluations, extractions, and other basic dental services for the elderly.
